A sterile intravenous formulation of Omeprazole for use in hospitalized patients or clinical settings where oral administration is not possible. It is indicated for the treatment and prophylaxis of acid-related conditions like peptic ulcers and stress ulcer bleeding in critically ill patients, and for severe GERD. The injection provides rapid and reliable suppression of gastric acid secretion, ensuring therapeutic effect when the oral route is compromised.

Indications
  • Erosive esophagitis (short-term IV)
  • GERD when oral not possible
  • Zollinger‑Ellison syndrome
  • Duodenal/gastric ulcer (IV alternative)
  • Stress ulcer prophylaxis*
Dosage / admin
  • Usual adult: 40 mg IV once daily
  • Zollinger‑Ellison: 60 mg q12h, individualize
  • IV injection: over ≥2.5 min (4 mg/mL)
  • Infusion: dilute in 100 mL D5W/NS, 20‑30 min
Use within 4h after reconstitution – no preservatives
Reconstitution
  1. Add 10 mL sterile water for injection
  2. Swirl gently – 4 mg/mL clear solution
  3. For IV push: withdraw dose & inject slowly
  4. For infusion: transfer to 100 mL D5W/NS

Do not refrigerate after reconstitution – precipitation risk.

Contraindications
  • Hypersensitivity to omeprazole / benzimidazoles
  • Rilpivirine (significant interaction)
  • Atazanavir / nelfinavir (reduces antiviral effect)
Warnings
  • Exclude gastric malignancy before use
  • Acute interstitial nephritis – discontinue
  • Hypomagnesemia with prolonged use
  • Avoid with clopidogrel (reduced effect)
Adverse reactions

commonheadache, nausea, diarrhea, injection site pain

serious rare anaphylaxis, SJS, pancreatitis, hypomagnesemia

Interactions
  • ↓ absorption ketoconazole, erlotinib, etc.
  • ↑ methotrexate, cilostazol, warfarin (monitor)
  • Avoid clopidogrel, atazanavir, rilpivirine
Storage / supply
Store below 25°C · protect from light · single‑dose vial

Hepatic: max 20 mg/day
Disposal: biohazard waste recommended. Discard unused portion 4h after reconstitution.
